Study Group Registration

ASSUMPTION OF THE RISK AND RELEASE. I understand that by attending an in-person IDOC Study Group Meeting: I may be exposed to infectious diseases including, but not limited to, COVID-19. While particular rules and personal discipline may reduce this risk of infection, the risk of serious illness and death does exist; and I knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19; and I hereby release IDOC, LLC, and its directors, officers and employees, from all liability and any claim related to my attendance at the IDOC Study Group Meeting, to the fullest extent permitted by law.
